SUBJECT: claim for damages – Costa Concordia shipwreck

The undesigned ___________________________, as passenger/s and holder/s of contract n° ______________ (enclose contract), given the grave events the occurred on the night of Friday 13th and Saturday 14th January, 2012, and that caused the shipwreck nearby Island of Giglio (Grosseto, Italy), I/we do hereby claim damages suffered because of the aforementioned events.
I/we also reserve the right to integrate and quantify such damages with further notice.
In particular, I/we declare I/we suffered:

• damages to persons (enclose documentation)
• damages to baggage and things (enclose documentation)
• patrimonial damages (i.e. cost of cruise, expenses incurred after shipwreck, etc…) (enclose documentation)
• damages from ruined vacation,
• moral damages.
  In case the above requests will not be considered, I will take legal steps.
